Co-Management of Heart Failure and Diabetes

Description – As the medication expert, the role of the pharmacist is to ensure that patients receive safe and effective medication therapy, get the most benefit from the medication that is prescribed, and achieve optimal outcomes related to their medication therapy.  Continuing education programs for pharmacists are required so that pharmacists are prepared to use evidence-based practices and guidance from experts as they work to identify medication safety concerns and to prevent morbidity associated with improper drug selection, sub-therapeutic dosage, failure to receive medication, excessive dosage, drug interactions, drug use without indication, and treatment failures.

The goal of this webcast is to improve participants' ability to make informed decisions regarding the management of medication needs of patients with diabetes and heart failure.
